Oakheart can make four attacks if it attacks using it's Tree Arms and two attacks if it fires wooden javelins.   Tree Arms
Melee Weapon Attack: +11 to hit, reach 15 ft., one target. Hit: (1d10 + 6) bludgeoning damage.   Wooden Javelin
Oakheart can shoot a sharpened stick from his branches: Ranged Melee Attack: +5 to hit, 60ft, one target. Hit: (1d12 + 0)   Juicebox
Oakheart makes a grapple check against a target. Athletics (+11). If you are successful you can make an attack with advantage. +11 to hit. Hit: (3d4 + 5) Piercing damage. You heal the damage you deal to the target as you drain their blood.   Animate Trees (1/Day).
Oakheart can animate 1 or 2 trees within the area. As an action Oakheart animates 1 or two trees within 60ft. They remain animate for 1 minute and are mindless, obeying all mental commands. You can use your bonus action to give a new mental command to the trees. If they do not have a command they will defend themselves but otherwise will take no actions.

Oakheart can use their reaction to intercept any attack that goes through their threatening zone or targets a creature within their reach. This cannot protect a creature from an area of effect.
